I keep reading the journals and notes in each scene hoping for explanations, but they all just hint at what happened in that room. They are helpful to solve the puzzles to an extent though so it's worth the replay to find all the items after reading all the notes. Sometimes I'm still clueless though and just click all over the screen until I find the missing items.
Through the various scenes there are optional dolls to find and disease pieces to find that disappear after finding them. These 2 optional collectables are for 3 extra scenes that are separate endings. The doll ending is the good ending then the other 2 aren't but the last one does recognize you did everything.
Overall, it isn't long, but the optional stuff is hard to find making it take a little longer. The story is interesting albeit confusing most of the time. I enjoyed it as a short game to take a break from longer ones personally. I'd say if you want to read a medical murder mystery and solve a few hidden object puzzles finding it on sale would be a worthwhile purchase. I'm not sure I'd pay full price though since it isn't long or exactly replayable... There are the optional endings and completion acknowledgement then you're done basically.
